SC/Chaplain helps patients and families along their path to spiritual growth, overcoming obstacles and putting pain and grief into perspective.
CHHA provides services that promote hygiene, comfort, and personal care.
MSW assists the patients and families to be connected to community resources.
A Volunteer offers companionship and support to patient’s families.
HOSPICE focuses on providing individual care plans to meet the needs and wishes of the patients to manage the pain, anxiety, and symptoms of the disease, as opposed to finding a cure and seeking aggressive treatment.
HOSPICE CARE can be provided at private residences, assisted living, senior residential homes, board and care homes, and skilled nursing facilities.
